Payment Options
Faith First Healthcare – Arizona
Serving Maricopa, Pinal, and surrounding counties
At Faith First Healthcare, we offer flexible payment options to ensure clients receive the care they need while supporting our dedicated caregivers. Below are the payment methods we accept and important information about our private pay policies.
Accepted Payment Methods

1. Arizona Medicaid (AHCCCS)
We are contracted with the following AHCCCS health plans:

Mercy Care
-
Banner – University Family Care
Once services are authorized by your health plan, we will work directly with your case manager to begin care.

2. ALTCS (Arizona Long Term Care System)
We provide services to individuals eligible through ALTCS.
Beginning October 1, 2025, we will also be contracted with:

Arizona Complete Health – ALTCS-EPD
Note: We do not assist with the application process for ALTCS, but we can refer you to qualified resources for help.
3. Private Pay
We welcome private pay clients who wish to self-fund care.
Accepted private pay methods:

Credit or Debit Card

Zelle
-
ACH/Bank Transfer

Cashier’s Check or Money Order (No personal checks)
Private Pay Policy Highlights:

A retainer is required to begin services. This is not a deposit but a prepaid amount held to secure your caregiver and schedule.

Payment for ongoing services must be made weekly or monthly in advance, depending on your care agreement.
Private Pay Retainer & Refund Policy
We understand that care needs can change. To ensure fairness to both our clients and caregivers, the following refund policy applies to private pay clients only:

No refunds are issued for services already rendered.

Refunds for unused service days are considered based on:

Whether services were ended suddenly or were planned and communicated in advance
-
Whether two weeks’ notice was given to terminate services
-
The caregiver’s ability to be reassigned to another client

Two-Week Notice Highly Recomended
We require at least two (2) weeks’ written notice to end services. This allows us time to transition your caregiver to another case without disruption to their income.

If we are able to find another client before the 2-week period, services may end on your requested date.

If services are ended immediately without notice, Faith First Healthcare reserves the right to retain the initial retainer to cover lost wages owed to the caregiver.
This policy helps protect the livelihoods of our caregivers, who often rely on consistent income. Sudden cancellations can negatively impact their ability to support themselves and their families.

Exceptions for Refunds
Refunds or early termination may be granted without penalty in the following situations:

Client passes away

Documented medical emergency or hospitalization

A verified issue with the caregiver or company that cannot be resolved

Significant changes in care eligibility (e.g., Medicaid approval with new provider)

DISCLAIMER FOR MEDICAID CLIENTS
If you are receiving services through AHCCCS or ALTCS, we are not permitted to provide private-pay services at the same time unless written approval is received from your assigned caseworker. Please speak with your care manager if you are interested in supplemental services.
