The Client Coordinator will facilitate communication between clients, caregivers, and management, helping to coordinate services and address any concerns. What We Expect

Responsibilities:


- Serve as the primary point of contact for clients, addressing inquiries, concerns, and requests in a timely manner.
- Conduct client assessments to determine care needs and preferences, ensuring personalized service delivery.
- Coordinate schedules for caregivers, matching them with clients based on needs and expertise.
- Monitor client satisfaction and follow up on services provided, resolving any issues that may arise.
- Document and maintain accurate records of client interactions, care plans, and service changes.
- Collaborate with caregivers and management to ensure high-quality care delivery and compliance with company policies.
- Assist in onboarding new clients and facilitating the transition into services.

Qualifications:


- High school diploma or equivalent; a degree in healthcare, social work, or a related field is preferred.
- Previous experience in a client-facing role, ideally within the healthcare or home care industry.
- Strong organizational and multitasking skills, with the ability to manage competing priorities effectively.
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
- Proficiency in using computer software and electronic health recor
d (EHR) systems for documentation and scheduling.

- Problem-Solving Skills: Ability to think critically and address client concerns or needs promptly and effectively.

- Availability: Must be willing to work flexible hours, including evenings and weekends, as needed to meet client and caregiver needs.

Additional Information

- Work Environment: The Client Coordinator will primarily work in an office setting but may also visit clients' homes or attend community events. It’s essential to be adaptable and comfortable in various environments.

- Team Collaboration: You will work closely with caregivers, Field Supervisors, and management to ensure a high standard of care is maintained. Building strong relationships with your team and clients is key to success in this role.
